中山大学药学院万国辉教授团队| 用“细菌纳米注射器”重塑口腔屏障免疫:一种面向未来的精准递送设想

SciOpen 口腔是人体最早接触外界环境的屏障之一。它每天经历咀嚼、摩擦、唾液冲刷、微生物定植和反复的微损伤,但在健康状态下,牙龈上皮、基质细胞、免疫细胞和口腔菌群之间能够维持一种动态平衡。许多口腔炎症性疾病,包括牙周炎、牙龈炎和复发性口腔溃疡,表面上表现为菌群紊乱和炎症加重,但更深层的问题往往是局部屏障免疫失去了“分寸”:该防御时反应不足,该终止时炎症又持续放大。   近日,中山大学药学院万国辉教授团队在Oral Science and Homeostatic Medicine发表文章 “Engineering bacterial nanosyringes for regional immune recalibration at the oral barrier” ,该研究提出未来口腔疾病治疗或许不应只停留在“杀菌”或“抑炎”,而应进一步走向“区域免疫校准”。   目前,口腔局部治疗已经有较成熟的体系,例如机械清创、局部抗菌药物、缓释凝胶和牙周袋给药等。这些方法对降低细菌负荷、控制局部炎症非常重要,但它们大多属于相对“粗放”的干预方式,主要解决的是药物能否停留在局部、能否持续释放的问题。相比之下,口腔炎症真正发生和维持的空间非常精细:上皮细胞、髓系免疫细胞、成纤维细胞、驻留淋巴细胞等不同细胞群,在牙龈、龈沟、结合上皮和非角化黏膜中形成了高度区域化的免疫结构。若要真正修复口腔屏障,仅仅减少细菌数量可能还不够,更需要把调节信号准确送入特定细胞,纠正异常的炎症回路和修复程序。…
